1.     One year Contract is effective from the date of the order. PolTV customers receive a 14 day trial period which commences on the date when customer orders PolTV services and on which PolTV charges the customer for the additional equipment, along with taxes and shipping fees. PolTV service may only be stopped within first 14 day trial period if Customer experiences technical issues. In order to stop the Service during the trial period, the customer must call PolTV at 1-866-400-4600 to cancel the Service before the expiration of the first 14 day from the date of the order. Any client that cancels his or her Service during the trial period and returns the receiver(s) to PolTV at 350 N. LaSalle Blvd., Tenth Floor, Chicago, IL 60654 within 30 calendar days of such termination in good and working condition shall receive a refund for the purchase price of additional equipment (cables, Wifi etc). If equipment is not returned within 30 days and Customer is charged for it than PolTV has a right to charge client a restocking fee of 25% of value of such equipment.

2.     If the customer cancels his or her Service after the 14 day trial period ends, the customer shall pay PolTV an early Service termination fee as follows: 

a.     If the customer has purchased a one (1) year Service Contract, the customer shall pay the greater of $100 or fifty percent (50%) of the remaining balance due under the Service Contract.

b.     If the customer has purchased month-to-month Service, the Service will terminate at the end of the month in which the customer notifies PolTV of such termination. The cost associated with the purchase of a receiver is non-refundable.

After the expiration of Contract, the customer shall automatically become month-to-month customer and such Service rates shall apply. Notwithstanding the foregoing, to the extent that a customer has leased a receiver, the customer shall continue to pay monthly rental fees on such receiver after the applicable Service Contract has terminated based on the rental fees paid by the customer during the term of the Service Contract.

3.     Any receiver leased by customer shall remain the sole property of PolTV. The lease payments do not affect PolTV's ownership rights in the receiver(s) and do not give the customer an ownership right in the receiver(s). All leased receivers must be returned to PolTV by the customer upon termination of Service.

4.     The use of the PolTV product may cause customers to exceed the monthly data transfer limit set by the customer's internet company. It is the Retailer's express obligation to inform all customers that the use of the PolTV product may result in the customer receiving a higher monthly internet bill than the customer received before using the PolTV product. PolTV is not responsible for any charges incurred by any customer due to a customer's use of the PolTV product. Low monthly data limits set by Customer's internet provider are not considered to be valid technical reason to cancel PolTV service.

5      All prices listed on the Order Form are subject to change in the discretion of PolTV and do not include taxes.
